Dr. Fazalare is a native of Weirton, W.Va. and earned his medical degree from West Virginia University School of Medicine in Morgantown, W. Va. and completed his residency at Hamot Medical Center, Erie, Pa.

He specializes and provides expertise in pediatric and adult athletic related injuries. He has ongoing research experience in a biomechanics study comparing arthroscopically tied knots and meniscal repair which was a current concepts review article.
